Medication Policy
Students are not allowed to carry medication to and from school. All medications need to be brought in by a responsible adult and delivered to the nurse. All medications require a doctor's prescription and a signed parental authorization, even OTC Over the Counter medicines.
I have and can give acetaminophen (tylenol) and benadryl as long as I have a signed OTC parental consent form on file. These are sent home the first day of school. If you did not get one, please let me know and I will send one home.
If your child will need a short term antibiotic to be given in school, let the pharmacist know and ask them for a separate labeled container to be left at school.
You will need to pick up any medications at then end of the school year. They will not be sent home with your student.

Below are a number of different forms including Food Allergy, Asthma, Seizure action plans as well a general medication forms. You may download and fill out the needed forms from below. You can have your doctor fill out the Dr form below or he/ she can use their own form/ prescription. These forms are also available on the District web site.
